THINK!! Before going to A&E Please contact your GP Surgery in the case of an emergency. The A&E department is under immense pressure therefore contact your GP Surgery first and we will endeavour to do what we can.

PATIENT CONSENT – Collecting Prescriptions From 25 May 2018


You will NOT be able to collect a prescription unless you are the named person. If you wish to nominate someone to collect on your behalf.  Please complete and sign the Consent Form Patient Consent- Presc

The practice will be closed for training on the first Tuesday of the month at 13:00.

 

Should you require urgent medical attention, please telephone the surgery on 0208 477 4317 and your call will be re-directed to the Out of Hours services

 

Prescriptions due to be collected on these days will be available from 08:30am the following morning.
